Lessons Learned From Linking ADAP Data with HIV Surveillance Data to Improve Health Outcomes in Massachusetts
HIV care measures for people with HIV enrolled in the Massachusetts AIDS Drug Assistance Program (ADAP) were compared to people with HIV not currently enrolled; results indicate that enrollees have higher frequencies of engagement and retention in care and viral suppression. Evaluation of the data provides insight that enhances programming and service delivery to reduce health disparities.

Incorporating Telemedicine into Care at a Large Urban HIV Clinic
Access to telemedicine visits in a multidisciplinary urban HIV clinic allows for improved access to care for patients who have fallen out of care or those who have difficulty attending their appointments due to stigma, multiple comorbidities, or barriers related to work, transportation, or childcare.

Driven by Community: A NYC Directive to Improve Health Outcomes for People of Transgender Experience
Transgender women, especially those of color, have substantially lower rates of viral suppression than people with HIV in general. The New York City HIV Planning Council collaborated with the community to develop guiding recommendations that drove the development of the jurisdiction's first psycho-social service directive specifically for people of transgender, intersex, non-binary, and/or non-conforming experience.

Implementing Evidence-Informed Interventions with Transgender Women to Improve Access, Engagement, and Health Outcomes
This workshop will explore the implementation of peer-led, evidence-informed interventions for transgender women with HIV. The programs are Healthy Divas, and Transgender Women Engagement and Entry to Care Project (T.W.E.E.T.). Attendees will learn the core elements of these interventions and how they improve engagement in care, treatment adherence, and viral suppression in Alabama and Michigan.

Utilization of Telemedicine in a Rural State
Oklahoma has been classified as one of the seven states with a high rural HIV burden. Telemedicine is an excellent resource to reach patients in rural areas and continue to engage them in care and keep them virally suppressed.

Presenters will describe a medical community collaboration to resolve care gaps and improve patient health outcomes through the co-location of health home care management by a community-based organization (CBO) within medical clinics. Increasing Access to HIV Care via Telemedicine at a University-Based Ryan White Clinic
Telemedicine has emerged to provide medical care to those in rural settings. This model has not yet been utilized by Ryan White clinics at academic centers. We use a multidisciplinary approach to conduct visits with patients via live videoconferencing. There are multiple challenges, but there is potential for increasing access.

Telehealth to Support HIV Prevention and Care in South Carolina: Three HIV Telehealth Pilots
Using three HIV telehealth pilot projects as case studies, telehealth leaders from the Medical University of South Carolina—a HRSA-funded Telehealth Center of Excellence—will discuss how telehealth might be leveraged to improve access to HIV prevention and care.
