Retaining Aging HIV+ Patients In+care

Review of a project that has experienced success retaining aging HIV+ patients in care, presented by Emily Tsuchida of CARES Clinic in Sacramento, CA. While aging and elderly populations are typically more conscientious in making and keeping their appointments, the complexity of managing multiple chronic illnesses makes retention in care all the more essential for this group. This session concludes with a review of data submitted through the in+care Campaign to date and an introduction of new enhanced data reports.
