Data to Care: Improving Treatment Outcomes & Addressing Disparities

Discussion of how programs can use HIV laboratory surveillance data to identify persons who have fallen out of care in order to guide re-linkage efforts. This is called Data to Care, under which projects use data to identify reasons for drop-off in retention to care (identifying patients lost to care, those who have transferred to new care or moved) so that programs can then create Data to Care outreach efforts.
