Description

The HIV Cross-Part Care Continuum Collaborative (H4C) is a new HRSA initiative to coordinate the efforts of all Ryan White grantees to improve viral load suppression through quality improvement efforts like data sharing and alignment of quality improvement activities. Seven states have been selected to participate: Arkansas, Indiana, Maryland, Mississippi, Missouri, New Jersey, and Ohio. The initiative started September 2013. H4C reflects the National HIV/AIDS Strategy's goals to use evidence-based activities to improve HIV/AIDS prevention and care in areas hardest hit by the epidemic and, additionally, supports implementation of the White House's HIV Continuum of Care Initiative "to accelerate efforts to increase HIV testing, care, and treatment to better address drop-offs along the continuum of HIV care and increase the proportion of individuals in each stage along the continuum."
