After establishing buy-in for PCMH development from leadership and staff, the next step is to become familiar with the foundation necessary to become a PCMH. Building blocks (engaged leadership, data-driven improvement, empanelment and team-based care) are foundational to further practice development for additional building blocks (patient-team partnership, population management, continuity of care, prompt access to care, comprehensiveness and care coordination and template of the future).
